WELCOME TO BIRCHWOOD CARE HOME
The entire staff at Birchwood Care Home welcomes you! Birchwood Care Home is a community of 60 residents, approximately 50 staff and many volunteers.
We realize the days before and after an admission are filled with many questions and concerns for you, as well as for your family and friends. We hope this booklet will acquaint you with our facility and provide the necessary information to insure a smooth transition. We would like to make Birchwood Care Home as home-like as possible and are constantly striving to serve you better. Our commitment is to provide quality care for our residents. In keeping with our motto: “Caring for Life”, Birchwood Care Home is proud to offer an environment designed to help maintain independence and individuality. We encourage support and attention from your family and friends.
Birchwood Care Home is dedicated to the following philosophy of care:
All residents are individuals and are to be treated with dignity and respect regardless of age, gender, sexual orientation, race, color, payment source, religion or national origin.
All individuals have the capacity to grow, learn and be as independent as possible and Birchwood Care Home believes in providing these opportunities. Residents, whenever possible, are encouraged to make their own personal decisions.
A resident’s privacy, confidences and resources will be treated with respect and discretion. Residents have many rights and responsibilities detailed in the “Residents’ Bill of Rights” section found in this handbook. We encourage you to exercise these rights and responsibilities.
We believe that physical, recreational, spiritual, and social health can be achieved with the resident and entire health care team working together to meet each resident’s needs.
HISTORY OF BIRCHWOOD CARE HOME
Birchwood Care Home opened as a 40-bed state licensed Board and Care Facility in March of 1966. In 1971 a 20-bed addition and recreation room were completed bringing us to our current licensure of 60 beds. Birchwood Care was federally certified to participate in the medial assistance program in 1974. In 2000, three offices, cold and dry storage, elevator and front lobby were added to accommodate added staff needs and allow for expanded care of current and future residents. Our facility has always been family owned and operated.
PHYSICIAL PLANT DESCRIPTION
Birchwood Care Home is a privately owned health care facility that has served Southeast Minneapolis and surrounding communities since 1966. Our beautiful, well-maintained brick building is designed to provide a warm and homelike setting. Located in a safe residential neighborhood, across from Bryant Square Park, residents enjoy attractive views of nearby homes as well as being located only blocks from Lake Calhoun.
Through physical design and décor, we have taken great care to create a comfortable and calming interior environment. Our three-story facility is carpeted, elevator-serviced, handicapped accessible, air-conditioned and has warm colors throughout the building. The attractively decorated resident rooms include bathroom facilities.
Lounges with cable TVs, and patios and balconies offer inviting spaces for relaxation and visiting. The cheerful dining room encourages resident to linger after a meal, a snack or a cup of coffee and socialize with one another.
